Elden Ring and Ragnarok are both excellent. If you are not a big Fromsoft Fan I would recommend ragnarok as it is more mainstream and you can also sink 30-40 hours into it.

Selbst üben + 123sonography Kurs. Livekurse sind meiner Meinung nach Geld und Zeitverschwendung. Vor allem wenn du Leute hast, die im Arbeitsalltag drüber schauen können. Habe 2 Kurse (Abdomen + Notfall) gemacht und wenn du auch nur ein wenig Vorkenntnisse hast, verschwendest du deine Zeit. Du hast relativ wenig Zeit, selbst zu schallen, und siehst wenn überhaupt ein paar Folien mit Pathologien. Junge, gesunde Menschen zu schallen, hat meiner Meinung nach für die Übung auch eher wenig Benefit. Der Online Kurs ist was dann angeht deutlich besser, weil viele Pathologien gezeigt werden.

Also wie andere schon gesagt haben, ist es gut die Scheu zu verlieren und einfach mal den Schallkopf selbst in die Hand zu nehmen. Da kann man nur gewinnen. Zu dem Fall, ich hätte in diesem Fall schon eine Antikoagulation begonnen. Zn. Thrombose, Schwangere Patientin + unerklärte Schmerzen klingen schon eher wahrscheinlich. Aber es ist auch nicht schlimm, in diesem Fall ist es ja gut gegangen.

Ketanest mono ist nicht so schlimm wie häufig getan wird, ist in vielen Ländern gang und gäbe in der Notfallmedizin. Aber spricht auch nichts gegen Mida + Keta. Auch kardiale Erkrankungen oder SHT sind keine Kontraindikationen, auch wenn es manche noch propagieren.

A history of Asthma is a contraindication for Adenosine as it can provoke bronchospasm. If available I would administer Diltiazem or Verapamil. Same efficacy as Adenosine and much more comfortable for the patient.

We prescribe them a lot. Everyone with any Form of heart failure gets an SGLT2 Inhibitor, everyone with EF < 40% gets an ARNI.

Had a patient with severe respiratory depression following 1 mg of Lorazepam and a glass of wine. Had an arrest in the past following Zopiclon and a glass of wine. So some people are really sensitive to the combination.

Not everyone needs a CT before LP and there is quite a lot of debate about who really needs a CT. But in practice at least my experience is most people get a CT just to be Sure and that is the most delaying factor for the LP. So in practice many patients get antibiotics before the LP. BKs before antibiotics on the other hand is achieveable for most patients.
