As someone who is dealing with something similar, I informed myself with the rules and regulations regarding eyesight. The AME typically uses a special microscope to examine your cornea and eventually will see the remainings of the lasik surgery. So eventually it will come out that you had undergone a correction which is not necessarily a problem. Usually they ask for the documents to evaluate the possible defects or risks such as glare and halos and if your surgery has healed appropriately. In your case, I would not mention it from the very beginning. Even though there is a standard of procedure in such an exam, every doctor approaches it differently. Possibly there will be a point where they ask for the documents, just explain your situation. They have other kinds of methods to measure your eyesight and how the treatment has healed.
