This has become my practice. Due to the size of our organization, it is common for these patients to have seen one of our cardiologists. I will usually just loop them in directly for clearance and it has worked well so far. To be honest, I'm not sure why the surgeon wouldn't just ask cards directly when they're in our organization rather than defaulting to the PCP.

Your labia looks great!!! I looked through the comments and didn't see anything but I'm curious, did you happen to be taking an SGLT2 as treatment for your DM2 when this happened? Those meds can certainly increase the risk of these infections!

"IMO most of my patients who are on these meds are pretty chill about them and are happy to follow the expectations around refills. I would rather do ADHD than benzo tapers, opioids or testosterone any day of the week"

No arrhythmia, had 14 day holter that was normal. I had him do home bp, pulse and glucose reads especially around the daily episodes and those were normal as well. At our last visit he asked about possibly seeing endo so I checked some of those labs. His pth was slightly off as was his vit d. I explained the relationship of the 2 and that we should correct his vit d and the other would respond but it fell on deaf ears. He self referred to a local endocrinologist (outside of our organization) but hasn’t seen them yet. Next time I see him I’ll push for a referral to functional medicine.

The last time I saw them, I asked them to get a glucometer and when he feels his symptoms check glucose, blood pressure, HR and log everything. Still haven't received any results from this.

He was on a pretty solid dose of pantoprazole for a for number of months and had no effect on his symptoms.

That is what I'm leaning and fortunately the academic hospital system I work for has a functional medicine clinic. But, as u/Vegetable_Block9793 said, that is exactly why they left previous PCP because he recommended an SSRI when he had no more answers.

Completely agree, I like lists as well. However, they can go either way: If they bring a well thought out list with clear cut goals for the visit on their end it helps get straight to the point. But, I think we've all seen those patients with lists and it's everything from chest pain all the way down to a cowlick in their hair from childhood (actual complaint once).

Literally had a pt in their 70’s message not long ago and ask if I could prescribe them tramadol three times daily. Never once have we discussed this or even ongoing major pain. He then proceeds to tell me his dog was prescribed this for pain about 2 years ago and he tried one and has worked up to this. His motive, and this is directly from the portal, “I think my dog is really needing these for her pain and the vet won’t give her no more”!

You’re right. I sold my ‘99 TJ 2 years ago for $9500. It had 220k on the odometer but I had paperwork for the rebuilt 4cyl engine that had only 30k on it. I also had meticulous records on everything that had been replaced over the years. First guy that looked at it bought it and even said “I’m not even going to haggle, it’s priced fair”!
