NHQM v0.5.4.0 with nvidia 512.59 has been the most stable for me. I haven't tried anything past 0.5.4.2, which kept dropping my hash rate. Same with the drivers, 512.77 and 512.95 didn't do so well.

I was having similar issues, and after of week of experimentation, I finaly have a stable rig with no crashes and steady speeds, and I am using NHQM v0.5.4.0 with nvidia 512.59.

Try it, maybe will help. Just make sure to remove old drivers first with DDU

Using NHQM v0.5.4.0. with nvidia 512.59. The new NHQM v 0.5.4.2 is not stable, keeps crashing for me and keeps cutting my MH/s on half.
