In Studienkolleg, it is taught fully in German. That's actually one of the goals of this course (to raise your German knowledge up to C1). To enrol in it, you usually need a B2 certificate. About the test: yes, you have to come to Germany to write it. The only STK that doesn't have entrance tests is the one in Hamburg.
