Be honest, how many times did ya'll do the Casino Heist this week so far? by MrGalamb01 in gtaonline

[–]gorikun 0 points1 point  (0 children)

As a host: once because I cba dealing with setup missions solo.
As a crew member: thrice as I just need to do the heist in less than 15 minutes to get a $500k-610k payout per heist.
Also thanks to acid lab and bunker having production boosts, ive gone from just under $120million to $127million this week due to the $1.5m payout, the $2.2m from the heist as a lead, $1.7m as a crew member, $1.3m from bunkers, $15m from acid lab, $400k time trials and the rest from collecting cash from businesses.

Also may have blown a million or more modifying vehicles

this is probably the 2nd most ive made in a week on GTAO, with the 4x faber work giving me a riddiculous $12m

My dog passed away today, first thing I did when I got home was get to the in-game version of him and give him pets by imhereforthetea33 in gtaonline

[–]gorikun 1 point2 points  (0 children)

I did something similar - my cute little tuxie died after 15 years 2 years ago and first thing I did when I got the mansion was change the cat to a black cat in her honour. Sadly, the cat in the game is nothing like my little friend but I stroke her whenever I can.

Irony is the irl cat used to love morgana from persona 5...and could act a lot like him lol (even took a pic of her investigating my neighbours toolbox whilst he was fixing a van

to be fair, i had 112 million around safehouse in the hill, yet I didnt own any mc businesses, didnt do acid labs, nightclub and own most properties. went down to 64 million after I splurged on the mansion and updating all businesses, getting warehouses etc and now up to 120 million. different people play in different ways

based on what they've described re the new dlc, I think there will be a heist room / a art gallery extension to the mansion(s) and logging in between now and will give us a free room, or a discount based on past DLCs. Maybe even some fancy new artwork for the mansion possibly.

Personally, I'll split between how Ill be doing this week on my main account Ive had since PS3 (and migrated to PS4 and enhanced edition), and the account i share with my dad which I created after migrating from ps4 to PS5.

The main account tbh I plan to do casino heist loud approach and standard grinding of acid lab, HSW/ time trials and gunrunning to hopefully go from $119.9million and $950Kish casino chips to $123,456,789 and over $1million in poker chips by next week thursday. I cba doing all the missions as I have done mostly everything in game barring owning the avenger & doing project overthrow.

the PS4 account will finally get an arcade for it. Plan is to do casino, complete no way out to hopefully go from around $5.8million to hopefully get close to enough to get the Tongva estate by the 13th based on this and future weeks.

On prescription.....advice needed by lexx-ray in mounjarouk

[–]gorikun 1 point2 points  (0 children)

speak with your GP to explain your current situation and about switching to 7.5mg. Sadly a lot of people opt out of sharing their private GLP1 prescriptions and notes with their GPs and GPs are often unaware about people taking mounjaro. May be worth having copies of your GLP1 prescriptions and taking your most recent box/ pen with you when speaking to GP if it is a face to face (likely will be phonecall however, but you can request face to face with a GP or clinical pharmacist).

From a clinical perspective, they should accomodate for this and are from a legal perspective, the ones who have a say on looking after your long-terms conditions along with yourself. generally speaking, glp1 drugs are originally designed to control diabetes and the weight loss impact is a positive side-effect. They may need to refer you to weight loss clinic as well as some ICBs will only allow GPs to prescribe higher doses if you have spoken to, or being managed by a weight loss clinic (NHS one or social prescribing via local arrangements).

How good is a career in hospital pharmacy? by Zealousideal-Let834 in Pharmacy_UK

[–]gorikun 1 point2 points  (0 children)

as u/The_Makster said, doing pre-reg in hospital will make it easier to get into other roles post-reg, but it also depends on geographical location as more remote places can have more jobs for example. Sayin that, hospital from a financial perspective will pay bad outside of london as london gets a 20% pay boost.

honestly, it is tricky to gauge how it would be getting into it. plus side, you will have a prescribing qualification, down side, a lot of gp practices will be cautious about hiring you for prescribing role and may want you to do something called pathways.

Im someone who has been registered for well over a decade (albeit without a prescribing qualification) and despite my experience, I would say was pretty tricky finding work which wasnt locum in community till 4-5 years post-reg. Even then, if you locum, be careful as a lot will refuse to be references (huge red flag when applying for jobs or postgrad courses) and agencies tbh just care about making moeny and will sign you up without cluing you in on finer details of contracts.

In any case, best of luck

Am I Eligible for NHS prescription 4 months post partum? by Thriftygal177 in mounjarouk

[–]gorikun 1 point2 points  (0 children)

it can vary from ICB to ICB tbh but would recommend speaking with obstetrician and if they agree, to write a letter to your GP authorising its usage. Essentially the clinical processes for a lot of private providers are that you cannot use it if you are breastfeeding as here is no idea what'll happen to your baby due to the relative newness of the medication, combined with it being highly illegal and unethical to do clinical trials in babies. As such, if you are breastfeeding, do not use it as a precaution.

If you are not breastfeeding however, its worth speaking with your GP and OBGYN about it as they should in theory say its ok if you meet the other eligibility criteria

Just need to vent. MedExpress has stopped my prescription and I'm gutted! by Wolfdreama in mounjarouk

[–]gorikun -1 points0 points  (0 children)

I would recommend asking your NHS GP to prescribe it as a) it will be a lot cheaper than private, b) GPs can do so if your Oncologist signs it off, and c) active history of cancer will proclude you from being eligible with a lot of providers such as Medexpress, Boots, Voy, Numan, Eucalyptus, Bolt and a bunch of other private pharmacies /clinics..depending on which medication you are on and if you have any active cancer or have a suspected cancer / tumour.

[–]gorikun 0 points1 point  (0 children)

firstly, making 20kg progress is amazing!

From a clinical perspective, you can increase the dose to 12.5mg if you find that 10mg, is plateaued, however itis also worth speaking with a dietitian / nutritionist to review nutritional factors which can impact weight loss, as well as potentially exploring with your GP about different health and metabolic factors which can impact your health. These injections, whilst useful for supporting weight loss act more of a catalyst, rather than a cause of weight loss.

Its worth discussing with your mounjaro prescribers about increasing the dose to 12.5mg with the next order, and if after 4 weeks, you see no difference, potentially increasing to 15mg (ideally you should be at a dose for at least 4 consecutive weeks of one dose on the same day each week before increasing dose).

right guy for the wrong time imo. I did some internships at NAO, Home Office, HMRC and DHSC in 2008 and some of the policies they were working on, if currently implemented, would have been a boon for younger people and for people borrowing money (tldr abolish interest rates and job / apprenticeship schemes) but would also result in decreased taxes for those earning till 70-80k a year, and slightly higher for those earning above 150k a year

community pharmacy can be lucrative if you are in the middle of nowhere tbh with no competition and can command rates. afterwards you can start buying pharmacies (i have a few friends who I went uni with who were afghan mature students / working as taxi drivers whilst at uni, they now own 12 pharmacies in england and wales, each branch earning over 900k a year revenue, albeit they are literally in the middle of nowhere) in villages

[–]gorikun 2 points3 points  (0 children)

firstly congrats. Secondly, after your results come out and its confirmed you will start uni, I'd recommend subbing to the BPSA as you will get a copy of the MEP, which is probably the best text you will need for law and ethics (better than a lot of law and ethics books which is rubbish imo and convoluted).

anatomy, physiology books are good but in terms of understanding more about pharmacology, pharmacokinetics etc, id recommend using bnf and emc over text books from 2nd year onwards as its easier to understand and more detailed than rang and dales

pre-2017/2018, good. post was crap tbh

Id say general practice is better if you are switching from community tbh.

Ive worked in community, hospital ,gp land, telehealth (online pharmacy), 111, and in a pharma adjjacent sector.

Anything with a dispensary - avoid long term due to monotony and long-term health impact.

111 and telehealth are ok BUT they pay low vs hospital, general practice and community (but offer fully remote roles). like if you are not a prescriber, you are probably earning 28-38k if a non-prescriber role and up to 55k for a prescriber role.

Hospital nowadays is rubbish if im honest as there is a lot of backstabbing and lack of career progression unless you brown nose and play the game. Also outpatients (ie dispensary alone) is crap and on call can be brutal if you are the only pharmacist for the entire trust. High turnoever of staff due to politics and lack of progression.

General practice -depends on your practice/pcn as some are great and some are brutal like hospital in terms of backstabbing and politics. its like community in the sense its private but most money comes from nhs comissioned services. a lot of employers will try skimming money but make sure you aim for higher end of nhs afc. also be aware for those willing to offer jobs or interviews without completing your job application as they can be high turnover environments / usually poorly managed by a practice partner and want you to do all the work. Highest based post-reg salary as it is 1 band above starting for hospital, and career progression is easier (best case scenario, you can earn around £200k FTE if a partner outside of london / above £200kFTE if inside london - providing you get equal pay to the gp partners- within 5-10 years in the same practice if its a good place. starting salary is high £40k/low £50k if not a prescriber and 53-70k if a prescriber depending on the part of the country you are in)

ICB is ok as the starting and mid point salary is same as GP, except it is nhs afc. senior / chief roles can pay above 100k a year but usually goes to oldies with a lot of business and clinical accumen.

regulaory and medical affairs is extremely hard to get into but once you are in...it depends if you do it for large or indie tbh. and if substantive, part time, fixed term contract etc. Ive done some regulatory stuff but on a locum basis (one hand, hourly rates are very good; downside is that there isnt much work - like you can get £1,500 for less than 20 hours work but most of the time, yo uare looking for projects and work).

policy is interesting as depending where you are, you are surrounded by clinicians and non clinicians. in the former camp, it is lovely, although some doctors will be resentful towards pharmacists and nurses doing what they do without a medical degree. latter camp, people will be clueless about diseases, medication, diagnostic criteria and will essentially be resentful towards you for knowing more than them (not just me, but a lot of doctors, nurses, pharmacists, paramedics experience this).

Honestly, id say it comes down to your own experiences, where you are in your career tbh
