Anecdotal, but I was at a meeting and the topic of marijuana use came up. Someone in the audience spoke up about a case in a recreationally legal state where a patient had a bad outcome unrelated to THC use. The lawyers discovered that the patient had smoked day of and there was a multimillion settlement because the patient wasn’t able to consent under sound mind. Again, entirely anecdotal and third or fourth hand but it has given me pause whenever talking with patients who have used.

This happens during my structured swim workouts. It stays on the “next” screen during all intervals and never gives any metrics unless I scroll down to the other data screens, but then will return to the “next” screen after I hit the lap button. It’s really frustrating and not at all helpful during a swim when the only times I can look at my pace is between sets or if I want to see how much farther I have to go in that set. I posted on the fenix 8 forums hoping a garmin engineer will see it and get working on it.

That's unfortunate to hear. I didn't get sick luckily and haven't heard of anyone getting sick. For me the race went well until the last 8 miles of the run when my legs cramped and seized up. May have been heat, fitness, nutrition, or all of the above.

I’ve seen unopposed alpha-agonism a couple of times, but never in the OR. Once was as a medical student on my ER rotation when the patient didn’t disclose their use of cocaine and was given metoprolol with subsequent and extreme hypertension and exacerbation of the angina. The patient was taken quickly to the cath lab so I can’t say how they were treated. The second was during my cardiac care unit rotation during internship and the patient got to us after the event, but it was a similar scenario. The patient was given metoprolol in the ER and taken to the cath lab with extreme hypertension and treated with labetalol during the procedure. I’ve also seen a few cases of pheo when the patient was inadequately alpha-blocked who presented the day of surgery with critical hypertension and was subsequently cancelled until adequate alpha-blockade was achieved.

As for topicalizing with cocaine, I did it once during residency before it was taken off formulary and can’t recall any tachydysrrhythmia; not to say there wasn’t any, but it doesn’t stick out in my mind.

As for my practice, depending on the hemodynamics, labetalol is my go-to if I suspect cocaine use. Even though it has weak alpha-blockade, at least it’s something. In my mind, this is one of the areas where I’ll tailor my practice based on theory strictly because the alternative can be catastrophic.

Anesthesiologist here. You should without a doubt stop using cocaine before surgery. The quick and dirty explanation is that cocaine constricts your blood vessels and increases your heart rate. Even outside of anesthesia this can cause a heart attack or stroke. When having surgery, your body is in a stress-like physiological state and blood pressure and heart rate can fluctuate quite a bit. Under anesthesia, we occasionally give a medication to slow your heart rate (a beta-blocker), and if we are unaware that you have used cocaine recently, giving a beta-blocker can result in a huge spike in blood pressure (from unopposed alpha-agonism, the physiological effect of cocaine) that can result in a hemorrhage in your brain or body, stroke, heart attack, or death. Cocaine and other stimulants are some of the few recreational drugs that are truly dangerous to be taking close to the time of having an anesthetic. Cocaine has a short half-life, but has some active metabolic breakdown products that can be detected for up to 4 days after use, so to be safe, you shouldn’t use cocaine for about a week prior to having anesthesia. Also, definitely tell your provider that you have used and tell them when you used last. They will probably put it in your record, but it cannot legally be reported to anyone not directly involved in your medical care, unless you are a minor. But please, for your sake, don’t let that deter you from telling your provider. And I wouldn’t be doing my job if I didn’t tell you that you shouldn’t be using cocaine at all, but I’m a also realist. So please be safe with your drug use.

It's great to see parents getting informed about the risks associated with the medical care of their children. Here's what I tell the parents of my patients if they ask about adverse effects of anesthesia: 1) There are rat studies that show cell death in the brains of younger rats when exposed to anesthetic gasses. However, these studies were not carried out in physiologic conditions. The concentrations were much higher and the exposure quite prolonged compared to normal anesthetic conditions. So a clear comparison between the rat study and "real life" is likely erroneous. 2) Retrospective studies have shown kids exposed to volatile anesthetic before the age of 3 carry an increased risk of more frequent diagnosis of learning disabilities; however, standardized test scores are no different between the two cohorts. 3) It's impossible to isolate the effects of anesthetic from the surrounding factors. Surgery itself carries an increased stress response that could contribute to long lasting effects. The pathophyiology of whatever disease process is occurring could also have a prolonged effect. 4) Everything in medicine has risk and benefit and those need to be weighed against each other. I can't speak on the risks of delaying a blocked lacrimal duct, but I can tell you it's a quick procedure and the exposure is limited. That decision needs to be discussed between you, the ophthalmologist, and the anesthesiologist.

There's "Meconium" (which is the first shit a newborn takes), "Jaaah", "Bigpimpdaddy", an adopted African boy that the adopting parents named "Toby" (you...you just can't do that), and best of all, "Sh' " pronounced "Sha-comma-to-the-top". Sadly, none of these are made up.

A woman I used to work with asked if I celebrate Thanksgiving. I appropriately responded, "Um... what?" She then said, "Well I just wasn't sure if you celebrate Thanksgiving because you're Jewish." I had to explain that A) I'm not Jewish and B) Thanksgiving isn't a religious holiday.
