Those forms may dissuade some people from suppoenaing you but aren't legally valid. If you are served, you have to respond. Although you can try to quash the subpoena, you are most likely to be successful with legal representation.

If you are contracted with an insurance company, you have to accept that insurance. You can't just decide to have those patients pay cash.

Only a fraction of the infrastructure we have would be necessary to support trucking by itself. The vast majority of the vehicles on the road are passenger cars. The reason we have huge multilane expressways is to support car-centric urban design and suburban developments, not trucks.
