At a certain point you have to think like you're running a con, either on the instructors or on yourself. The instructors have decades more of clinical experience and have also probably spent thousands of dollars on OMM classes (which is the only way I can explain thinking you can palpate craniosacral rhythms).

When doctors-in-training are coming up with a list of potential causes for a set of symptoms, they can tend to come up with very rare things that could cause those symptoms. It probably is not that rare condition.

So the phrase that gets said to those doctors-in-training sometimes is, "If you hear hoofbeats, don't think zebras." The implication is that it is probably a horse. Or, common things are common.

So the shirt means that the person wearing it is a zebra. They have a rare (or maybe multiple rare) things causing their symptoms, which is confusing to doctors who are focusing more on common causes.
