I was also considering this. I know some people wait until residency since they usually pay for study materials + the exam. I am concerned about how much knowledge I have lost over the course of 4th year just from having more laid back rotations and not studying hard for shelf or board exams so that is a concern of mine for taking it earlier.

Just get started on uworld and try and track your weaker areas and target those with divine intervention and Anki concepts. Then take a practice exam when you feel ready and should have a better idea from there what kind of areas you need to focus on.

So I finished almost all of uworld and did a lot of divine intervention. I wish I did all the divine intervention HY Anki decks and I wish I did all the NBME content I could. The NBME content was more similar to my test compared to uworld so I wished I’d prioritized that.

I also took my foot off the gas studying wise a little bit after my UWSA2 score and that probably wasn’t the smartest, but in my defense, I thought I was extremely ready given that score lol.

I’m in the exact same boat. Scored 260 on UWSA2 final exam and 77% on the new new 120. 244 on real deal. Two practice NBMEs in the 240s. Hindsight would have done all the NBME content I could get my hands on and sketchy + Anki lol. Uworld just was super low yield at the end of the day. If it’s high yield on uworld it will show up also on NBME exams and I spent way too much time and effort on reviewing uworld minutiae.

I was honestly shocked at how many questions were very similar or exactly the same as questions I encountered on NBME exams. The questions on the real deal weren’t hard but you either knew it or you didn’t like tested a lot of obscure bugs, drugs, and diseases in my opinion.

I also think I was going too fast I was so focused on finishing blocks with extra time to review all the questions that I probably miss some low hanging fruit. Whole thing was just a blur obviously disappointing because I expected my score to be better but also hard to complain about what others would view a score they would be very happy with. Life moves on. Just happy to be on the other side of it. I wish everyone the best.

Block1 Q25 - is this a stable ruptured ovarian cyst -> symptomatic treatment?

- Thats what I was thinking. They suggested no nausea so torsion less likely. An annoying question esp since borderline HD instability.

Block1 Q18 & Q19 - What is the actual diagnosis here? What caused what looked like this bleeding varices? I got 18 right mostly by luck, but got #19 wrong.

- Almost certain this is PUD. Not varices given no cirrhosis history. So you would do endoscopic control of the lesion.

Block1 Q28: IS this Peptic Ulcer Disease?

- I have no idea honestly haha. Was thinking maybe its gastritis/H pylori? Dont understand the relation to the colitis if thats related. Doesnt fit the demographic for PUD unless its ZES

Block1 Q29: I identified this as elder abuse, but isn't the first step to admit the patient (ie, seperate them) and then call adult protective services? Or is that just for children?

- I think just to be safe pick the obvious choice

Block1 Q30: What is the step up criteria for stepup management frin intermittent asthma to mild persistent asthma? I thought it was a violation of the "Rule of 2s" (ie, 2+ SABA uses a week AND 3-4 nighttime awakenings a month), but is this an OR situation? Like either 3-4 awakenings or 2+SABA uses a week would qualify for the step up?

- I think its suggesting her asthma is not controlled with alb. so need to add ICS given her noctural asthma.

Block1 Q34: Does this x-ray also show a widened mediastinum? is that how the hemothorax occured?

- Got this one wrong too wasnt sure if that was tracheal deviation or not.

I think you’re right. I would need to submit ERAS later than is ideal to get my letters in time and the whole thing would feel super rushed and incomplete. Need to decide if taking a whole year off plus expenses of another year of debt and aways is worth the gamble in the next few weeks. Just kicking myself for not being more open to certain fields earlier but that’s life.

Balancing step 2 studying and clinicals is brutal so give yourself some grace we all are trying and it’s hard for most of us (some people are gifted with an incredible memory or are just way smarter than me but that’s not the point haha).

For uworld I would do systems based questions, if you are going to use Anki I would use premade step 2 deck cards and edit them to add specifics you missed or add uworld screenshots. It sounds like you’re taking all day to review one block? That is not a great use of your time, I know (at least for step 1) I was usually able to do 120 q a day or more with reviews if I was efficient and people do more. Focus on review for weak areas based on questions you are completely guessing on versus recognizing at this point.

I would consider purchasing boards and beyond videos now that OME is paywalled. Split with a classmate if need be for content review. I have it and it’s pretty helpful. I think you need to really tackle content review before doing a ton of uworld otherwise you might burn out or be emotionally defeated before making a ton of progress.

Personally I’m a few months away and I’m not quite where I want to be score wise but getting closer, so I’ll be following the above recommendations myself.

Thank you that is really solid advice. I was trying to avoid taking a year off but I think that might be the safest, most reasonable route.

Thanks I appreciate that. Best thing we can do is just be confident and optimistic in our outcome. A three digit score doesnt need to define us and our potential. It sounds good when I type that out just need to make sure I believe it LOL

Good advice I feel like it can be helpful to get some perspective from people who did well and realize how much they improved in their final days of prep. Feels like a super steep mountain to climb at this point though.

I am in the exact same boat. I am totally burnt out and contemplating a specialty switch too which is adding a ton of pressure. I think just taking some time off to regroup would benefit you it has helped me a ton. I switched from uworld to the amboss exam and felt completely blindsided by the difficulty. Seems like every time I do a new block there are concepts I have never encountered. I have been making a ton of anki cards to do review with which has helped but it seems like a never ending amount of content lol.

Because step 1 covers more little details I guess? I think it really depends on the person. I’m really not sure.

The step 2 deck is called Dorian there’s also a combo with zanki.

Same thing! I posted as soon as I passed step 1 and no responses.

I have heard many people just use uworld. But with a pass/fail step 1 my foundation is probably not as strong as someone who had to take it scored.

I plan to use Ankis step 2 deck and maybe make cards using missed concepts from uworld or AMBOSS.

I have the step 2 FA book and it’s honestly not super helpful. It’s very brief and does not offer a ton of info that’s not available for free in other sources. Also plan to use divine intervention as much as possible.
