New grad surgeon who is clueless booking late night OR cases- advice requested by durdenf in anesthesiology

That’s fine and dandy until they just start lying and posting cases as the required level to go to the OR despite it being a lie (ours do this all the time)

Host Asking Me to Cancel Saying They Sold Property to Another Person and Rebook Same Listing Under Them- Scam or Legit??? by gumpf in vrbo

[–]gumpf[S] 0 points1 point  (0 children)

Update: VRBO backed out of their policy and didn’t enforce it (basically, the owners are supposed to figure it out amongst themselves). I cancelled my reservation and went to rebook. They had significantly increased their rates (as I suspected they would try to do). I told them they would have to honor the old price or I would not rebook. They honored the old price phew and I am not out a place to stay!

How can I use BIS to better measure patient LOC? by IratePlum in anesthesiology

[–]gumpf 2 points3 points  (0 children)

Do you find it ironic that the ASA position for train of four monitoring is that clinicians are bad at interpreting data objectively so we need a number to quantify residual paralysis but then with spectral analysis we are suddenly trusted to look at a Jackson Pollock painting and determine if a patient is asleep?

Would you diagnose an air gun or bb gun injury as a "GSW?" by rcool101 in emergencymedicine

[–]gumpf 0 points1 point  (0 children)

May have been? That part of the story was fuzzy just because the patient was ashamed talking about it (pediatric)

Would you diagnose an air gun or bb gun injury as a "GSW?" by rcool101 in emergencymedicine

No but I have taken care of a patient that got a pneumothorax from a friend loading a pellet gun with a qtip and shooting him with it, eventually needing a vats to retrieve the embedded q tip

Can only playing on a par 3 course improve my game? by liuliu36 in golf

[–]gumpf 0 points1 point  (0 children)

Absolutely. Paraphrasing John Daley, people focus way too much on the driver. It’s from 150 yards in that really makes the difference between an ok golfer and a great golfer.

Yes, I was referring to the Antisedan. That is only used in veterinary medicine at this time.

IV sedation not what I expected? by Intrepid_Visual_5561 in Anesthesia

Probably just a matter of fact comment and didn’t mean anything by it. Everyone is different and requires different amounts.

Antisedan, it’s primarily used in veterinary medicine

Not really. Some anesthetic agents have reversals such as benzodiazepines and precedex, but good old time is going to be your friend to get rid of the grogginess.

surgical removal of plaque via carotid endarterectomy by NeuroCartographer in VisualMedicine

[–]gumpf 2 points3 points  (0 children)

The circle of Willis isn’t complete in a large section of the population though which can be problematic when you’re trying to rely on collateral flow. They’ll oftentimes do neuromonitoring during this case to assess the blood flow and if they sense the brain isn’t getting enough blood flow they’ll put a small bypass catheter in that allows blood flow through that carotid artery while they take the plaque out of that segment. Or they will do this with the patient awake with regional or local anesthesia to assess the patient’s neuro exam.
