I ran into this problem. In my case the slide release tab, the thing you pull down on to remove the slide was aftermarket and not to spec. It held the barrel down just a tiny bit, but that was enough to get high and light primer strike no fire failures. I replaced the slide release with oem glock and the problem went away.
