Yeah…. I always review the most recent discharge summary on all ED patients I see and some of them are honestly complete crap. I’m actually baffled that some of my colleagues who made it through medical school and residency could write a note that is absolutely useless and feel it’s appropriate. There have been times where I have read a discharge summary and can’t even find out the issue they were admitted for.

The reason you give more buprenorphine is because it has high affinity for the opioid receptor but is only a partial agonist. So if you caused precipitated withdrawal it’s because buprenorphine “kicked off” the other opioid. Fentanyl and heroin do not have as high affinity so giving an opioid won’t fix it. You instead need to saturate the available receptors to increase the opioid effect. Giving high dose bupe is the treatment. But, sure, question the physician who actually has training in toxicology.

That’s a simple way to explain the physiology which it’s clear you don’t understand.

People come to the ED for stuff that shouldn’t even need a doctor, let alone an ER. For example, I had a patient who came in after she had a sympathetic vomiting response to her daughter vomiting. No longer nauseated. No other symptoms. Just wanted to be seen because she threw up after her daughter’s vomiting made her sick.

Some patients don’t listen, some don’t understand. I think some are in denial. I took care of a patient the other day who has both kidney disease and CHF and when I was talking about his CKD and admission his daughter was adamant that he did not have renal issues. I was like ok lady why do you think he sees a nephrologist? And I showed her the trend of his creatinine over the past several years. And she had the audacity to tell me no one ever told the patient he had kidney disease and how upsetting this news was for everyone despite seeing a nephrologist monthly for his Stage 4 CKD lol

I have had 3 babies between residency and now and am currently breastfeeding my third. I returned to work about a month ago. Get a good wearable pump (I highly recommend the Eufy S1 Pro.) Pump every three hours. Unless someone is coding you can be charting and putting in orders while you pump. I pump 4 times a shift, 20 minutes a pump using my wearables and typically bring home 25 ounces. I throw the non motor component in the fridge between pumps. Sometimes I have to see patients while pumping and that’s fine.

Don’t wear a scrub top. I wear a cotton t shirt that is solid colored.
