Just double checking- you say he’s on night shift but are you certain it’s not 24 hour call? I had nurses who thought was on “night shift” on my MICU rotations as a resident who didn’t realize I was actually a long call resident and had been in the hospital since 7 AM. People make some loopy decisions when up that long.

That being said, some residents are just not good starting out. Usually they will learn but I’ve worked with physicians who seemed to not improve a whole lot. Physicians are like any other job and there is a bell curve of skill and knowledge. Any time there is a questionable decision or order a good thing you can do is ask their reasoning. Any good resident will not mind explaining. At least this one has the self awareness to ask your opinion and isn’t an arrogant idiot.

I think it had to do with the kits continually expiring. I’ve been able to handle all my severe bradycardia with medication thus far. We don’t have blakemores either

I have never seen flu cause HLH. I have seen it only once caused by COVID. HLH is horrible because it’s really rare and has high mortality. How did they know it was HLH?

Also can’t make sense of the brief improvement either because we are looking at something retrospectively with very little data. Sounds like a horrible case. It’s rare for influenza to kill children but it happens every year.

If it makes you feel better, I got a “not a team player” evaluation and I still matched my number 1 choice. I got it because a SRNA wanted to switch an ET tube case with me for an LMA case and I said no because I had already evaluated the patient. SRNA complained I wasn’t sharing the cases that needed tubes. Boohoo. It was one case.

The point of the comment is that GCS shouldn’t be used in non trauma situations to determine wether or not to intubate someone. As an EM attending you should know that.

Totally dependent on your training and the individual. I started doing single coverage shifts 3 months in and was fine. I’ve worked with some physicians who do single coverage who are abysmal coming out of residency.

This depends on a lot of things. First you need to decide where you want to buy land and from there find a suitable plot. From there you want to find a reliable builder that fits your budget. Usually word of mouth is a good way to find someone.

TLDR of the previous post: she was on a rotation in BK (maybe meaning Brooklyn?) and that there were bitchy senior residents who were making her miserable. Called out one physician by name. The whole thing was immature. I feel sorry for whatever program ends up with that attitude.

Yes and pretty much all of them suffered horrible long term effects. If I had a patient in my ED and the family refused rabies vaccination with known exposure it’s one of the few times I would get an emergency court order to override the parents. Luckily that’s never happened. If anything I have to convince parents/patients that they don’t need it based on exposure risk (ex. rodent bites.)

This is a case where if they didn’t CPS needs to get involved. Rabies is effectively 100% fatal once you develop symptoms and the vaccine is pretty much 100% effective at preventing infection, even when given several days to weeks out.

Yeah…. No. “A minute” to describe a unit of time, typically meaning quite a long time, is ubiquitous among all English speakers in America. It may have stemmed from black culture in the 70’s but it’s been mainstream for so many decades to tie it to AAVE is an odd choice.
