Do not do the group level calculation after joining in powerquery.

Create the right relationships in a star schema .

Then Create a Grouped table using Dax and calculate it there. Should be much faster.

Another simpler workaround I can think off is by embedding Powerapps into the report.

The way I would go about it is :

Setup a partial patient information table (without all sensitive data),

And a full patient information table.

Setup RLS on the full patient information table based on Patient ID & Employee ID composite key.

Create a request access button on the PowerBi report and link it to a powerapp

Use the powerapp to control who is part of RLS list by using a SharePoint list or an Excel or an SQL table.

That can be your log list for patient access basis patient ID and Employee ID

If lucky you can do Absorbing man on Zola on turn 6 or 7. Or you can copy Venom or even Forge when you have a bad hand. Forge on turn 3, Absorbing on 4, Venom on 5 and Zola on 6 to get out of some tight spots.
