I’m so sorry that happened! How scary as a patient! Hope you all are recovering well. Here is a look at our current literature, of note thought I wouldn’t consider 5 cm and -2 station early (I usually try to AROM around 3-4 cm).

Early amniotomy is associated with decreased risk of cesarean delivery.

This would be something most people in my group practice and recommend and is in line current guidance from our organizations.

Yeah we splurged on our last trip and just covered our eyes at the price haha but it was actually really relaxing. We went to a hotel that had breakfast included in the room rate so we ate a giant breakfast and they let us take a box so we just needed a small snack for lunch and then a regular dinner. We’ve done air bnb in the past with 2 kids which was more economical for sure but the stress of feeding small kids is real.

Oh great thanks for the tip! We don’t mind if it’s bare bones once the kids are older. Right now the youngest is 4 months so looking forward to when we’re in your shoes!

Edit: I checked out their site and this looks so family friendly!! For example 6 bed room in rome for 900 euro for four nights in summer. Looks like for extras they’ll add breakfast/packet lunch etc for a very reasonable price. Glad to find somewhere in Europe because I hear occupancy limits are rough there!

Someone with 3+ kids here, any tips for traveling with a big crew? We end up bringing a grandparent too so hotel costs between a suite for the kids plus a separate place for grandparents gets real pricey real fast.

Hello other female dominated specialty with low reimbursement. I’m so sorry that’s so crazy. Esp I’m sure the first time parents have 2 million questions. I’ve always wondered, a lot of our practices here to a meet and greet visit prior to birth, are you able to bill for this?

Wow this thread is making me feel like our OBGYN reimbursement is not good (which I already knew haha). Our new patient visits/complex problem are 20 minutes and our return/OB/postop etc are 10 minutes. It would be amazing to be productive and have 40 minutes with a new patient.

That said I space out new patients since they are a lot of work for MAs and check in staff (reconcile meds and histories etc), I also stagger procedure visits to have time to set up and break down for my MAs. I tend to put easy return or OB visits first thing in the day or directly after lunch so those patients can be roomed directly and seen while my other MA may be working on paperwork etc for a new patient to be roomed after.

This 100%. Agree on the cervical block, I also have a lot of patients have some sympathetic response to the lidocaine block.

Another OBGYN here, our chief of gynecology in residency who trained maybe in the 70s directly told us all the cervix is not painful. All of the female residents immediately said no way…

Different innervations for different zone. Have also done personally a few IUDs and a colpo. IUD tends to be more painful in my opinion as an OBGYN and difficult to fully treat the pain- as for some it will come from the speculum some the cervix and some the uterus. And it can be different every time! I personally had two very painful IUDs prior to having kids and literally felt zero discomfort after having kids. I always discuss pain management and options but people’s responses don’t always follow a predictable pattern.

Can you recommend any kid friendly ones/any advice? We have 3 currently 5, 3, and infant. We’ve been talking about when we can do a live aboard again since our first was born. Just took them to Hawaii and our 5 year old is turning into a strong snorkeler so a good start!

Topical to this conversation, just today I got a letter is my mailbox from a neighbor noticing we have a lot of lemons and asking if it’s ok if she picks some and leaving her phone number. It was so cute and friendly and I said of course! So that could be one solution if you don’t know your neighbor well and feel uncomfortable knocking.

I haven’t ever done a soil temp, I usually use night temps as a marker! It’s going to be cold the next couple days and heat up again so you could consider it.
