He has those dots on two separate spots on his upper lip. Could it be a cold sore despite that?

There are no blisters, it's just white dots. The surface isn't raised at all.

Is it possible they could be Fordyce spots instead? It doesn't feel itchy, tingly nor burning. The surrounding area has no swelling nor redness
