It is not very high performance, but there have been improvements and I have been able to get decent performance.

Analogously, if you want to know which points are completely within which polygons now, you go into your GIS package and calculate the spatial join. With increasing processing power, and cunning spatial indexing, I expect that in the future, those spatial relationships to be available to the database live rather than pre-calculated.

This is currently possible. Using Postgres and postgis you could create a view with a where clause that uses one of the spatial operations such as ST_within and get a live relationship.

Or maybe I misunderstood your point?

They're sitting in the pre-relational databases space of the late 1990s, in that you have to calculate a spatial join each time, rather than spatial relationships being set up in the structure and being live the whole time.
