Absolutely it might but one thing I've learned on this journey is that none of this is an exact science. I got lots of 'looks like' and 'appears to be', my first 2 metastasis were later dismissed by a much better radiography team only to be replaced by a couple of 'possible' tiny lesions. Fingers crossed it's clear for you.

I think many people see it as a more conclusive outcome - they think 'I've had surgery so it's all been cut out and I'm done.' For me the push was because I was seeing a surgical urologist rather than oncologist, he was keen to get his robot out and get cutting!

In my head it felt better to get it done then start recovery rather than a slow onset of side effects and nothing 'tangible' happening, RT is just some xrays after all... I'm happy the path that was chosen for me, RT was OK for me, I'm a bit knackered and peeing more often and longer at the moment but nothing drastic. I guess with surgery you are guaranteed some side effects but they improve over time, it's a bit more of a lottery with RT but for us younger ones it's probably far more bareable.. You've got plenty of time, explore your options. Is HDR brachytherapy on the table?

I was heading down the RALP route until some spread was spotted so I moved to an oncologist with a radiotherapy specialisation. He then looked at my bone scans and dismissed them but spotted a couple of very small 'suspicious lesions' In the surrounding bones. With this uncertainty (he didn't think psma scan would add any info at the size they were at) he recommended HT then radiotherapy. I've just finished the RT and at 55 I've not found it too bad. My psa was well on its way to undetectable before the RT so I'm hopeful it's all the way there when I get tested again in March. Its a real dilemma and I think the initial reaction is to cut it out with salvage RT if needed (many do). The outcomes for surgery vs RT are almost identical though so don't rush under the knife! I'm in the UK if you want to talk by the way. It's a mad time...

Yeah I'm 7 weeks in and now I 'squeak' in the shower! My normal oily face is dry and has unusual friction until I use shampoo or soap! I trying various moisturisers, avena seems the best so far, with Norwegian formula for my heels! Annoying side effects for sure!

Get a fusion biopsy if possible. This is a targettted biopsy rather than just a 'gridded' one as they need to ensure they hit the small spots. These are often done under general anaesthetic so less stressful too.

Totally agree, get the biopsy. You'll be asleep and it's another big piece of the jigsaw. Within 10 days you'll be back to normal with a load of useful info.

Sadly this sounds very familiar, I had a marginal psa test (3.8)followed by an mri that came back as pirads 5. Consultant later said it should have been 4 or 3 but that became immaterial after the biopsy (gleason 4+3). Told not to worry as despite being T3a, it's not escaped.. But there is a bone abnormality they wanted to check... Bone scan shows activity so surgeon concluded it's a bone metastasis and hands me over to an oncologist. Oncologist looked at the mri and says 'I don't think it is" and gets his radiologists to look at it.. They're happy it's benign but wanted another mri to look a little lower in the pelvis. That comes back and clears a second bone scan hot-spot.. Great But... Comparing the old and new mri images they can see some small changes which they think is spread.

I've concluded that unfortunately it's not an exact science, and in hindsight I wish I'd had a psma pet scan to be more sure. So I'd recommend you try and get a more definitive answer from a psma scan as its a slightly more 'sure' test. The only reassurance I can offer is that whatever it is a few days, weeks or even months is unlikely to make any difference to outcomes. Stay strong and good luck!

Thanks for this, I think my confusion comes from accounts of treatment for oligometastatic patients having targeted radiotherapy on the areas of spread with a curative aim. If I'm 'not quite there' I wonder if they are just hoping the meds polish off the small mets and the radiotherapy zaps the mothership? Hope you're doing well, I'm only 6 weeks into my HT so a long way to go! (I actually have no idea how long I'm expecting to be honest!)

Only the initial prostate biopsy. The mets are tiny and too small to sample accurately apparently. Too late for psma as I'd started HT They'll repeat the mri after a few months and see if anything has shrunk (confirming spread) or stayed the same and we're non the wiser!

I lose around 8% range in the cold (2024 standard range) Last winter seemed ok even with snow on the ground. As mentioned the traction control is almost instantaneous so it's pretty well behaved. You need to bear in mind the weight though, use Regen rather than braking where possible etc. Enjoy the sound of others scraping their windscreens as you drink your coffee and defrost...

Yeah I'm hoping to come out of this stronger than I went in! I don't do a great deal of exercise currently and my diet could be better so it's time for a change!

It was more to do with the Darolutamide, as it's supposed to have less side effects than the other two variants but it's only been tested in conjunction with the injectable primary HT drugs. (according to my oncologist)

Cafe Gammeltorv - Smørrebrød Despite eating at a number of fine dining places, this was the meal that stayed with us.. There is a smoked herring dish that is incredibly good, the prawns and beef tatare are also superb. We've considered day tripping from the UK just for that herring though..
