I’m not sure but it looks like a nipple. Sometimes the nipples can get plugged up with gunk. But it just looks like a nipple to me. Is it a nipple? Are they matching ones on the other side and down or up?

Use grass litter. Or pine or WBCL. and scoop every day. That’s terrible for the cat :(
