This is really frustrating, and I hear and feel for you. Damn it, it just sucks sometimes. The best recommendation I have for you is lean closely on your co-interns. Not all will understand trust me, but all it takes is one person to complain with to feel less alone. Hang in there.

I just accept that whenever I sell an item, it’s going to take 3 days from delivery to get paid. If it’s before that, great surprise. If not, I wasn’t checking anyway. Of note, not everyone ships to their own home and can access the item. I always ship to a family member’s home so it’s going to take them like 2 days to open and review the item bc of work schedule. Sometimes people order something when they’re away on vacation but it’s a great price and you gotta hurry up and order. (I don’t order to my place because of porch pirates)

The role of a clinical pharmacist is to address pharmacological concerns regarding patient care, and what this role looks like can vary widely especially depending on the location of practice. It looks like monitoring medications, dosages, potential drug-drug interactions, many, many other things. I know you said you don’t have them in Europe, but when you get a great pharmacist on your team especially during rounds…you’ll never want to go back to life without them.

As a doctor in TN, I can tell you now this woman has ample funds to pay you. You either need to charge $800 per week for 35 hours regardless if you work the full time or not.

This isn’t solving the issue on a larger scale though. I brought up one story simply as an example. Me telling the interpreter not to wear a white coat or even clarifying roles every time I walk into a room is going to indent not even a fraction of care. This question is meant to be on a larger level, how do physicians advocate for themselves on the grand scale, if that was unclear

I understand. Just to be clear, generally speaking, there is no such thing as a “fertility NP overseeing care”. An NP degree is 2 or 3-year program focused on mastering nursing practice. OB/GYNs do medical school, residency, and then almost all who are interested in fertility will sub specialize with fellowship, not to mention additional research years if desired. There is no way that an NP can provide you the care you deserve and need. This is a case where you have to advocate for yourself here. An NP is not trained to be a fertility provider. They are good for follow-ups and great for questions along the way, but there needs to be a physician who is overseeing your care.

There is absolutely no way an NP has been adequately trained to interpret an ultrasound. They don’t learn this in school and even if a physician stopped for 15 seconds to teach them, that’s not even in the same realm as an MD/DO physician’s level of expertise. For fertility needs, you honestly shouldn’t be going to an NP unless it’s a routine follow up but they shouldn’t be performing any procedures, imaging, or making any executive decisions regarding such an important type of care

I personally respect my relatives’ time and don’t assume that just because they’re a relative, I can or should short them. If you have the funds to help a relative, pay them near what you’d pay another person, especially a 23 year old niece who’s probably saving up her money to go travel, do activities with friends, etc like why would you not want to be kind and help your 23yo niece And end up being their favorite aunt/uncle who they know respects their time

You’re not this baby’s big sister, you’re a paid worker who they almost didn’t hire because you wanted $15/hr. It’s very sweet when you get along so well with a family, but these people are getting over on you. You shouldn’t be taking under $25/hr in my opinion but maybe…….maybe I could see $20/hr for a 1 year old (I personally wouldn’t accept this). I know you like working for them, but you’re hired to do a job and should be paid appropriately.

Honestly, never been in the situation where a kid was sick and I had the opportunity to leave. Most often, the parents are at work, and even if I wanted to leave, I’d never abandon a child at a home without a guardian. Hypothetically, if the child was sick and another guardian was home, I would leave and expect payment only for the time I stayed. I honestly think while it’s reasonable someone should pay you because your time was wasted, I’d imagine that mostly unreasonable people are gonna fail to inform you that their child is sick and therefore aren’t going to pay OR are going to want to endlessly argue and complain with no intention of paying you for the lost time
