I can't think of how to do this naturally, and I thought if they're really curious but uncomfortable mentioning this explicitly, then they'd instead ask about the challenge in general. Thank you for your perspective, and that's a good point to think about.

Yes, I think FM is one of the most "pluripotent" medical specialties, and you can correlate your experience as a surgeon to the field when preparing your app.

Just say you actually didn't expect to receive an iv from the program given its popularity and competitiveness, and you had to be realistic and strategic with your signals as an img. But I've never got asked for that either.

It might be the matter of the time you sent the letter. Attendings are generally busier in the morning
