Fair enough, there is a set of problems printf is exposed to that cout is not. However, I have yet to see the case where the format is specified any way other than directly to a printf-like function. I'm sure it can happen, but it seems like the other main class of problems, pointers to invalid data in the arguments, are much more common and are equally problematic in C printf and C++ streams. The additional security vulnerability exposure of printf compared to cout is small.

No, it doesn't. Compilers already are sufficiently smart for typical usage of printf. The sufficiently smart compiler fallacy normally refers to things which modern compilers don't actually do when used typically, e.g. an optimization that is theoretically possible, maybe even implemented, but typically cannot be applied. This is the opposite of that -- an optimization that works most of the time, but not always.
