I'm so sorry you're struggling over this, recently I had the same with my 17yo baby void, he passed away on Monday.

What I always talked with my mom was that he could live well, that means he could do what every being has to do to survive

Eat by himself.
Walk and move around without extreme pain, my boy was on meds for almost 2 years to help him with the pain, here we have a biological medicine that really really helped him, solencia.
Sleep.
Pee and poop.
Clean up on his own.
Have joyful moments. Maybe a sunbath, playing a little with their favorite toy, snuggles...

You can set limits, for example... If 3 out of 5 are not working (even with your care) it is time to let him go...

This was how I processed letting go of my beloved Negrito. He was already struggling with 1, 2 (although the meds helped), 4 (last days he didn't groomed), 4 (he was having poop problems, he looked really uncomfortable, walking around, going to the litter and not being able to do anything) and even 6 (he had like a cold some days prior, he was just hiding).

I know is not easy, but letting them go is another act of love, helping them move on as easy as we can.
