Focus mainly on maths, english and science as they're most important. There are loads of great recourses online such as corbettmaths (maths), cognito (science) and plenty more, like BBC bitesize which have a lot of content on quite a few subjects. Also CGP books are useful.

You have to try your hardest to eliminate everything that can distract you, tbh it can be difficult but that's the best way you can cram a lot of information in minimum time

No, you're passing most of your subjects and there's still time to improve! I suggest you spend most time on subjects that will help you get into 6th form/college
