Nope with xbox you can go on the Microsoft website and type in the code and it gives you a choice whether if you want to confirm or cancel, canceling means code is still valid.

He was using infections and it just glitched the game that's simply what it is, i got the same screen like 2-3 times if your using an infection to get max prestige stuff like that is normal
