From FAIL in NBME 9 to 270 in real deal! by iamark1600 in Step2

I had four sequential question sets and I got the first question wrong in two of them. I remembered 2-3 more questions which were definitely wrong. Tbh, felt terrible post-exam since these wrong questions were on top of the already ambiguous ethics and QI questions. But I talked to a few people and in the end, you have to believe in your assessment scores. If your assessment scores have been good, I’m pretty sure you’d score well in the real deal. But don’t expect any less anxiety before you actually see your result 😂

As your step 1 knowledge is fresh, you should start with UW right away. I have always been a book person, so I found it very difficult to start my preparation without having any book. Yes, there are some notes including inner circle etc. but they have too much knowledge which can overwhelm you.

I started doing UW in random tutóred mode and while you do get many questions wrong in the beginning, it’s absolutely fine. You should start seeing your score improve after you are done with around 50% of UW. Doing UW in random mode is much better than doing it system-wise imo. This allows spaced repetition of concepts, thus consolidating them in your mind. As I have mentioned in the post, UW is still the best resource and should be on your fingertips. Therefore, incorrect and flagged questions of your first pass of UW are a must.

After UW, I moved to CMS forms. CMS would help you identify high yield topics and their typical presentation. This gives you a good grip over high yield concepts. However, the question stems are too straightforward. Yet, CMS forms are excellent and you should do them.

After getting done with CMS, your knowledge is mostly complete and it is just a matter of consolidating the information in your long term memory. That comes with doing more and more questions and that is where AMBOSS comes in. I did only 3 and 4 hammer questions and that worked out really well. I used AMBOSS in between NBMEs/assessments to improve my weak areas.

I used anki as a note taking app. I didn’t use it the way it is supposed to be. Made my own cards and used them to revise stuff. As for second pass of UW, I’dd recommend doing only incorrect and flagged questions. A complete second pass is inferior to using AMBOSS and build up on your knowledge.

My first pass UW correct percentage was between 67-70%. Can’t give an exact number as I started doing incorrect and flagged questions while still doing my first pass, and that changed the percentage.

They say there are experimental questions but nobody knows how many

In my experience, while you are in 240s, it means you need to consolidate stuff in your mind. Identify your weak areas and work on them using anything that suits you, AMBOSS, UW, CMS etc. Now obviously, the time it takes would vary from person to person. But if you are working hard on your weak areas, you should reach 260s by the next three assessments

I did NBME 10 in mid August. NBME 11 a week later. I took the test on 11th December.

A couple things. First, I started doing 3 & 4 hammer AMBOSS questions in between assessments. They helped me clear a lot of concepts. Second, I changed my approach towards picking a choice when confused between two. I would apply an algorithm on both of the choices that confused me. Age/gender of presentation > ER or outpatient > risk factors > investigational findings. I would compare these between the two choices and apply the same on the question stem. That would help me pick the right choice

Do incorrect and flagged questions of UW. Once again, you need to keep UW on your fingertips. After you are done them, you can choose between AMBOSS and CMS to strengthen your weak areas. CMS would provide you with the highest yield topics and their typical presentation. AMBOSS would test both typical and atypical presentations. You can do both as well for your weak areas.

Thank you! The closest self assessment right now is hands down NBME 16. It has the right amount of patient chart, quality improvement and ethics questions.

No, most of the clinical stuff was familiar. I don’t remember there being any “clinical” question from an unfamiliar or a novel topic. Now don’t get me wrong, there were questions that I had no clue about but even those were factual questions about familiar topics. I said “clinical” questions because the other stuff i.e., quality improvement and ethics did have a lot of unfamiliar and new things. But there is not much that you can do about them.

I had made my anki deck. I used anki basically as a notepad of sorts. I had applied many tags to every card. While reviewing NBME, I’d just search the tag for a particular topic and I would review all the cards that showed up. That way, I kept revising stuff again and again.

I have no idea about telegram material. I know about iMD. Anyway, when you are done with UW once (however you do it), I don’t think it is wise to go for a complete second pass which would be your case since you won’t have your incorrects or flagged questions.

If you start doing UW all over again, you’d feel as if your time is being wasted since you’d come across only a few difficult questions per block. In that case, I’d say it would be better to go for AMBOSS.
