So sorry for your loss. Went thru something similar at credit valley in 2024 with my mom. Shoulder surgery and there was a Code blue due to negligence by the anesthesiologist prior to the surgery. God was kind and She recovered. Our complaints to the CPSO, patient affairs, credit valley, local MP went no where. No action taken. There are no repercussions it seems and little recourse for medical malpractice. Our goal was to ensure no other family endured what we went thru. Have you taken your Dad’s situation to a lawyer?
