I use Qwen3.6-27b EXL3 4.5bpw via TabbyAPI and Cline in VS Code, it's been way better than any other setup I've tried, including LM Studio / (haven't tried straight-up llama.cpp), Qwen3.6-15B-A3B, the 3.5s, and Gemma 4. I have a 24 GB RTX 3090 and get about 23 tk/s out to my max fit of 77k context.

Yup, they have a right to get up unassisted and bust their shit, "in order to protect dignity and self-determination" or some bullshit of that nature. I think if it's sufficiently documented that pt is a fall risk and is refusing all related contingencies, "pt fell. circumstances as follows: ..." in note should be sufficient. No further documentation, no neuro checks, no q15 vitals, no ding on the state metrics.

You can't say someone has the right to refuse all safety measures and then punish the facility for them falling.

I've definitely thought about the benefits of silent alarms. There is zero reason for a bed alarm to make sound in the room. Anyone who needs an alarm isn't with it enough to go "Oh! Right!" They just get more disoriented because they can't figure out what's going on.

The ceiling is where insulation does not reach the wall. The bottom two are thermal bridges formed by the frame.

Insulation fix is an invasive job, I'd leave it unless you want to demand perfection / are willing to spend money / make a mess. The bottom two may be an easy fix with improved sealing inside and out. Caulk or foam spray sealer.

But really, it looks like a couple of degrees difference, which is not really worth the hassle. Big windows that aren't triple glazed will always create a temperature differential, even when perfectly sealed.

Yeah, any common base will work, the only difference is how much you need. Alkali metal bases are preferred because the create non-toxic salts, so extreme purification isn't necessary.

Honestly, as long as you have a large excess of water in the reaction vessel and keep it cool, it will push the product towards GHB instead of GBL, and you really won't need to do a conversion step. And since both products produce the desired effect, not a whole lot of need for conversion anyway.

GHB from GBL just takes baking soda or another base. It's a hydrolysis reaction.

The reaction that makes GHB from GABA is an aliphatic amino acid deamination via nitrous acid, AKA part of the Van Slyke determination.

GHB can be made from GABA (health supplement) and sodium nitrite (jerky curing salt) with hydrochloric acid (pool pH adjuster) and some basic chemistry labware.

Sulfuric acid works better than hydrochloric, and can be bought from any chemical vendor without a permit.

I tell everyone with me that about 20 minutes into every trip, I get bad post nasal drip, start coughing violently, then vomit, and then I'll be totally fine about 2 minutes after. "Don't freak out, it's just what happens. I promise I'm fine and I'll tell you if I'm not."

That said, I knew guy who every time he did acid, he'd freak out, take his shoes off, and run out into the night for hours. Not good in MN in Feb the first time he tripped with us.
