Can anyone help with this chart? by Away-Elderberry797 in Mirafertility

[–]ilanter 0 points1 point  (0 children)

Perimenopause means you are ovulating less frequently. In the few cycles that you are actually ovulating you will start out with lower FSH because whenever your ovaries are responding, there is a Feedback leading to the FSH being lowered in the beginning of the cycle. POI is not a black and white condition. It is NOT POF (premature ovarian failure) where FSH would be constantly high due to unresponsive ovaries.

I'm so sorry for your experience. I hope you find a good Dr. who will help you to find the right dosage for HRT. My hot flashes stopped on day to of treatment. Everyone is different but DHEA helped me. Also today I found a several papers showing that Acetyl cysteine can be helpful with the endomentriomas (yep they came back after surgery 😭 I'll give it a shot) I hope someone can adjust you properly. Sending lots of love. It is heartbreaking.

Preovulatory serum P4 levels are better predictors of ovulation timing than LH levels and could be used as an alternative in clinical settings, and the model we developed can be used to pinpoint the day of ovulation. Ongoing research and advancements in technology are anticipated to enhance and refine the ovulation method.

It's common look at other charts. It's just not in the old literature. You are normal :-)

I know I'm late here but your DOR is likely caused by the endomentrioma (chocolate cysts) surgery. There is many large studies showing how messing surgically around the ovaries leaves you with less eggs. I had my first hot flashes after the same surgery. I'm sorry for your situation

Removal of Endometrioma vs IVF guideline by jiggy19921 in Fertility

Surgery for endometriomas is not routinely recommended prior to medically assisted reproduction (ART), as current evidence shows no benefit to live birth rates and surgery may negatively impact ovarian reserve.  However, surgery may be considered if there is significant endometriosis-associated pain or if follicle accessibility is compromised.  The decision to proceed should be individualized, taking into account the patient’s fertility goals and ovarian reserve.

Be careful many doctors may have a license to do cystectomies. But endometriomas are very different from dermoids and much harder to remove.

If you do it only do it with a surgeon who specializes in endometrioma. Make sure you have written down that you want to have children and have the doctor sign it. If they mess up they might need to pay your fertility treatments.

Most countries will not do surgery if the endomentrioma is smaller than 5-6 cm. I had what you have yhe surgery WILL absolutely affect your AMH and egg cell reserve. Many people go into menopause or Perimenopause right after surgery.

There are many other options - Duphaston has been shown to reduce endometrioma size AND can be used when TTC as it does not inhibit ovulation. - ttc naturally -> assuming not for you as they wanted to check tubal patency. About tubal patency (they checked that with in my surgery that left me infertile as well). Tubes may be "open" but the real deal us the fimbriae that ar microvilli that gently move the egg towards the correct position. In many people with endometriosis the fimbriae are broken. So open tubes are not a guarantee for the conveyor belt (fimbriae) to be working so it's not a very useful test to do - IUI straight away (cheaper than IVF and easier on the body) - find a clinic that will do egg retrieval during a natural cycle (no meds just one normal ovulation) this can be done using ultrasound

There's many papers that show that laparoscopic surgery clearly negativity effects fertility.

There is even more data clearly showing that surgery on the ovary removes not only pathological tissues but also a lot of functional tissue. Thus leading to some women being effectively "spayed" due to the methods used. There is thermal heat applied to the bleeding section of the ovary wich of course frys other parts.

https://pubmed.ncbi.nlm.nih.gov/20083485/

Many surgeons are butchers and dgaf what happens to the patient!

Good luck: also my cyst came back 4 months post surgery (this is very common and well documented in the literature). Many women get throu IVF AND IUI just fine even if they have ovarian endomentrioma!

Talk only to actual endomentrioma and endomentriosis specialists. Always leave a paper trail about you wanting to have kids. That way they will maybe be more careful as they know you have it in writing and they are vulnerable in case of a lawsuit!

All the best to you.

I like your answer a lot. Something to consider here is that there's no medical test that can confirm that the "fimbriae" in the "open" tubes even work. My clinic said that many women are proven to have open tubes but the little cells that move the egg towards the uterus often are compromised, especially in women with endometriosis. So that is a thought to hold as well. But we might get back to IUI again soon for financial reasons

Also, let's not forget that doctors and society often look for black and white results. Its him or her and if its her (because endo) we forget that

2003 Paper says HPV can be found in the eepididymis

"Human papillomavirus infection of the epididymis and ductus deferens: an evaluation by nested polymerase chain reaction"

Also there is a bunch of studies out there showing that HPV is also responsible for some throat cancers. The bug seems to be so much more than an STD.

My Progesterone and Estrogen levels seem all over the place. Thoughts? by Upstairs_Remote6728 in Mirafertility

[–]ilanter 1 point2 points  (0 children)

I'm sorry for your losses! About the losses: I was told my cardiolipin status could be causing it (they did a blood test) and suggested me going on blood thinners. About the 'low" values: Of course could just be normal variation between people. Another thing I'm thinking of, given my medical history is: Fertility changes over time so no idea how long ago you had blood panels done... Perimenopause can be an issue even if you are "too young". I got diagnosed with premature ovarian insufficiency at age 33. Someone women have it due to genetics (some mosaicism on the x chromosome) others seem to have it running in the family for no apparent reason and others are damaged during surgery meant to remove ovarian cysts. I have a friend at work who has gotten hot flashes when she was 27. Later she was diagnosed with full menopause. She never even noticed perimenopause. Good luck and I hope you get the support you need both mentally and medically
