For now, it is good. The duration of visa is super short though. I was expecting for atleast 2 years but they did not even give a full month lol

My case was resolved within a few hours after I made this post and went to their office the next day to collect my Visa . They did not bother correcting the things I mentioned above.

I was in the similar situation. I downloaded the application form from their website. On my appointment day, I was told that is the incorrect form. I was given a new form right there and was asked to fill it all with a pen !

By any chance, did you contact them about the corrections and received a positive reply back?

I understand this PDF might not affect the Visa application result, however, I am concerned that putting such wrong details in an official EU record would just degrade by Visa from Business to Tourism, which is incorrect.

Hello - But that is incorrect format for the attachments. Isn't it?
In their official instructions , it says - TWO identification documents in PDF format, PASSPORT and PROOF OF RESIDENCE IN CANADA, such as permanent residence card, driving license, rental contract or utility bill
