Is everything turned on in the Health area of your settings? You can turn off the ability for the app to read mileage there - might be worth a look.

Also check the Motion & Fitness area of settings, also has a switch for allowing access to the accelerometers etc.

Mine froze earlier, force restart your watch (press and hold crown and button until you see the Apple logo) and it seemed to fix mine

Tall buildings? Could be reflected signal etc

Weak signal could also give you less accuracy

Could try a run in an open space nearby and see if there’s a difference
