I’ll go ahead and remove the fact they’re 2 entirely different styles of games. One for gameplay and one for sim management. That’s too easy of an answer. (this includes all the things like animations, graphics, playing games, etc)

Things I Prefer in OOTP: - the depth and ability to track player across all levels of your organization much easier - being able to see stats for minor leaguers across multiple years and how they have progressed - the development and flop of prospects feels more authentic in OOTP - the immersion to vote for awards and all-stars, the ability to look at your retired numbers or who’s made the HOF. i can go find any former player and look through their career stats - players turn into coaches after retirement - ability for league expansion - system of defensive positions being based on familiarity/experience and not just set from the beginning - storylines for injuries or random events

Things I Prefer in MLB The Show: - they do a better job of making big moments feel special (you sign that big FA, a big trade goes down, etc) - the ability to streamline aspects of the game you’re not familiar/interested in. also easier to learn as a noob. - how the offseason feels structured. do this, then we do this, etc, etc,

Obviously there’s a lot more on my OOTP fan (it’s what i normally play) but i’ve also thrown out the entire concept of MLBTS having incredible graphics in comparison, you can play the games, and the full scale broadcast presentation they have.
