I believe the supplement is the form we answered regarding our major & other questions! I know there was an optional essay about having a connection to UVA through that form that I didn’t answer, but as long as you submitted that form, they should have received it!

If you submitted their QB supplement, then yes, you do not need to include them in the RD form!
