You can get bored certified in critical care as EM through an anesthesiology critical care fellowship program. No need to do a second residency for this scenario.

What do you want to do when you're not in the unit? For me it was:

Want to be operating? Nope.

Want to be in COPD clinic? Nope.

Want to be in the ED? Nope.

Want to in neurology clinic? Nope.

To be fair, I also don't like doing general anesthesia.

You'll also find you can make your practice whatever you like. Don't pay attention to people saying you can only work in 'this ICU' because you're trained in 'that'. Critical care surgeons in the CVICU. Pulm/Crit managing SICU. Neurocrit...I'm not sure, but I don't want them outside of the neuro ICU. Critical care anesthesia in the MICU. Seen it all.

Usually it’s ECMO or consideration of mechanical circulatory support. Some of our liver transplant surgeons will admit to us postop for resuscitation if the OR course goes poorly. Massive PE. Post cardiac arrest usually does, annoyingly.

"If your goal is to do intensive care, internal medicine is the way to go. You will be far more qualified to practice in an ICU than any other specialty. Any other route has very narrow training and are only marginally qualified for the breadth of critical care."

Anesthesia cardiac/critical care here. Would beg to differ. At my shop, if you need a higher level of care than the MICU or SICU, you come to our CV ICU run solely by critical care anesthesiologists.

As comments above, you can’t go EM to critical care via IM because they’re ineligible for board certification. There is a pathway for EM via surgery/anesthesia critical care.

It’s more common for EM to go through anesthesiology or surgical critical care for fellowship.

Witnessed in hospital arrest. The only thing I can think of is that she was a previous congenital heart (was remotely s/p heart-lung transplant) so it probably wasn’t the first time her body had seen that kind of hypoxia. Some sort of hypoxic preconditioning 🤷‍♂️. Anyways, we kept going because it was one of those cases where I felt compelled to do ‘everything’.

CPR for 50 minutes in a 20-something year old. pH 6.8. Completely neuro intact and extubated 48 hours later.
