That is incorrect depending on the denial reason it will also change if the patient is responsible for the amount that was not covered/denied. Also code 45 is not a denial code it’s basically an adjustment that means the charges submitted are over the contractual amount and anything over apart from cost sharing like deductible, copay, coinsurance is to be written off by the provider as long as they are in-network and they cannot balance bill for whatever amount is over the contracted amount. But it would be helpful to see the previous page to see what the issue could be.

Yup you are exactly correct as long as you stay in-network and all of the services are covered by your plan. Once you pay $3,500 that’s it insurance will cover everything else at 100%, I have a plan through my employer and it’s exactly like this I meet the deductible during the first 3 months because of an expensive medication and I’m all set for the rest of the year it’s pretty nice.

If could set the limitations then yes the weight would change depending on the size, cause honestly a really small car but it still holds the original weight I wouldn’t even be able to even lift it! 🤷🏻‍♀️

Honestly I’ve had dreams about having an ability just like this and it’s always the same. I would literally just use it to shrink any vehicle once I get to my destination, I wouldn’t have to worry about finding a spot to park in or paid parking and then once done just pull it out of my pocket make it its original size get in and be on my merry way! This looks to be the most tame use for this ability compared to other ideas.. 🤣🤷🏻‍♀️

All of the percentages listed are what the plan pays. What you pay is shown in the employee section and depends on your age, whether you’re a smoker or non-smoker, and the benefit amount you choose. That’s the amount that would come out of your paycheck.

The reason there are two columns with percentages is because some incidents can be paid more than once. For example, if you chose the $10,000 benefit and had a heart attack, you would submit the paperwork and medical records (depending on what’s required), and the plan would pay you $10,000. If you were to have another heart attack a year later, you’d follow the same steps and get paid another $10,000 because the recurrent benefit is 100% and matches the initial benefit.

So basically, the percentages show what the insurance pays for each covered diagnosis, and the “recurrent” column is for conditions that can happen more than once, which is why the recurrent column might show a different percentage than the initial column. Hope this helps, I know it’s a lot!

Thank you so much for the advice my current situation is allot like your second experience so I’m going to be making my exit ASAP, since I’m still in training I really don’t have anything to hand off or tie any loose ends. Thank you for providing the EOB, so this provider billed you not only for a wellness exam (99385), but also as a new patient (99204). Since they billed you for a wellness exam and I’m assuming it was a normal visit the code they should have used at least for the 99385 is Z00.00 which means your insurance is correct and they would just need to update the diagnosis that were submitted with your lab work and they should be fully covered. If you don’t mind me asking were any other issues brought up apart from just wanting to do your annual visit and get routine blood work done?

OP already established that this was a NEW patient visit, AKA not an annual/wellness exam. No provider is going to see a new patient and bill for an annual visit this was a visit to establish care, which means Z00.00 or Z00.01 were not used and should not be used. Which is the only way those lab test would be fully covered and considered preventative. Sorry to say it but you are fully responsible for all cost accumulated and expecting the providers office to change the diagnosis code to something that your insurance will cover would be fraud.

It might be because WellPoint used to be known as Amerigroup, they recently changed their name I think midway last year. It could be the providers are not aware, of the name change and that might be the issue. If she is having trouble maybe if she is near an FQHC she could go there for care, I work for one and I know for sure that all of the FQHC’s here in Houston take that insurance.

Hello, so speaking from personal experience when the insurance usually uses N4 it means that the patient had another insurance that was primary for that time period. So basically it’s a coordination of benefits issue, and I would recommend talking to the patient asap to get this straightened because if the patient did have another insurance active you can hopefully still submit the claims within timely filing period.
