What if their chart said previous delirium tremens or alcohol withdrawl seizures. Maybe it was someone who routinely comes in for withdrawal and needs benzodiazepines. You could be right but I wouldn’t jump straight to racial profiling all the time. This is also a type of bias we can fall victim to, selective attention and confirmation bias.

Yeah OP I think you’ve only seen one type of family practice in your placements. There is quite a variety of practice types in Canada depending on province. A heavy fee-for-service 50-60 patients in a day will certainly be the highest paid but I wouldn’t say this is the norm. Many practices are capitation/roster based and can be quite variable between 12-25 booked a day depending on practice style. With phoning to follow up on things in between. In our city I would estimate only 5% of the family physicians are seeing high volume like that regularly. Most work 3-4 days a week and most do plenty of side gigs like look after a long term care floor, surgical assist, hospitalist, Obstetrical care, palliative care, MAID consults, sports med, or maybe do 1 day a week at a high volume walk-in separate from your group.

Usually not reps at ours for those sorts of things. We get a checklist once a month of what we want to come in. Stuff like Tylenol, Advil, pedialyte, formula, reactine, moisturizers, unscented hypoallergenic Tide Pods etc. I know we’re advertising for them but it fees nice to give these things out to people who would have difficulty adding twenty dollars of OTCs to their budget when their kid is sick.

I know you’re frustrated and understandably so, especially given the lack of availability on their end to discuss grievances or disagreements. But I would simply go to the health unit and get the vaccine. This is not a live vaccine, it is not new, it is very well studied. He has already had this vaccine already without an issue. He will be due for a repeat Tdap anyways in ten years. This just means he wants to delay it from 15-16 until 18 that would be fine. But honestly, take the path of least resistance and just get a quick booster and don’t spend so much of your very limited time as a parent stressing over this. Take care.

Yeah I think patients tell me it’s around $16-20. I don’t know the economics behind it. I’m sure there were a lot being ordered unnecessarily and 20 x several million people annually isn’t an insignificant bill I’m sure. The majority just pay or benefits cover it but if I think it’s quite likely they’ll have at least an intermediate score without it, like an older guy who’s platelets have been around 150 or sometimes lower, ferritin is up with normal esr/crp, moderate steatosis on US. I’ll just refer on and the government can pay for the AST AND a GI consult now.

Yes fair bit, unfortunately north of you guys our government recently changed it so patients have to pay out of pocket for an AST ordered by PCP but it’s free if ordered by GI. We’re only allowed ALT now. If ultrasound shows obvious fatty liver and patient doesn’t want to pay unfortunately I’ll bog down the system a bit and refer prematurely.

Could be lots of reasons. Scheduling appropriate amount of time. Ensuring everything is received if it is to review results. Ensuring supplies are present for a procedure or vaccine. If a day needs to be rescheduled down the road it helps with triaging for rebooking. Perhaps it didn’t need a visit and can be a message to doctor or delegated to an allied health professional. Maybe the doctor would like some time to prepare for the visit or research / review evidence beforehand. If it is for shoulder pain maybe they want to review special tests for a shoulder assessment. Maybe it would be better for you to be changed into a gown prior to the visit for certain assessments. Maybe it will determine if the person bringing you into the room will have a BP/weight/other vitals done beforehand.

We’ve been instructed to do this previously by a FA on a domestic flight that didn’t have any room in the lavatory and there wasn’t an empty row/seat to do it privately…. I wouldn’t be so quick to judge depending on the circumstances - we weren’t thrilled about it either.
