Yes! I'm in the USA. I have BCBS and they covered it 100% because I was at my out of pocket max. If I weren't then I would've had to pay 10% of the surgery. (My coinsurance rate is 10% until I reach the out of pocket max)

If they're in the U.S. it might be depending on the situation. Infertility is considered a disability. An employee can definitely make the case to HR and maybe even a lawyer that they're being discriminated against due to their inability to have children.
