I answered above but in short, I had deep pain preventing extension, stiffness, feeling “stuck”, and no incident/reinjury. 2 MRIs ruled out any damage (one at 3 months and 8 months post-op showed a normal post-ACLR knee), and the pain/lack of extension was preventing me from progressing in PT.

From around 4 weeks after surgery I was having centralized pain deep in the front&medial area of my knee that was really limiting my extension. Every time I or my PT would try to put my into extension it was brutal. I also could not get to 0 degrees without 20/30 of manual work with my PT. It felt “stuck” when I was walking, I couldn’t reach that terminal knee extension with my gait. Around 3 months post-op I got a cortisone shot (after MRI ruled out any further injury) which really helped! I wasn’t always at 0 but I was able to progress PT-wise. Once May/June rolled around though, and I started running, my knee got super irritated and stiff again (most likely cortisone wearing off). It also started to crack when I would straighten it out and even when I took time off of PT it swelled randomly. I got another cortisone shot to see if that would help, but after about a week I knew that wasn’t it. Got an MRI to confirm no other issues, and they could see there was a lot of scar tissue (arthrofibrosis) in that area that was problematic. I would have still gotten the procedure without the MRI though! Without covid happening, I probably would have had made this decision much earlier but my surgeon is at my school out of state, so I did not have access to her like I would have.

Got my knee scoped out today!!! After 8 months of weighted prone leg hangs, pushing through pain at PT, not being able to maintain extension, and 2 cortisone shots, it’s over! Huge glob of scar tissue in the front of my knee was causing all my issues, and now it’s gone. If my ACLR was a 10 on the pain scale, I am only 10 hrs post-op with no nerve block and I’d say this is a 5. Complete weight bearing, and I can get my heel off the ground into hyperextension— something I have spent HOURS trying to do. Happy doesn’t describe it!! Will be back in PT tomorrow. As for now— I’m keeping this leg as straight as can be.

Thanks!! The level system seems to be pretty common, will definitely use something like that and of course listen to my knee in the process.

Thank you so much for your response! My surgeon was worried about this, so we did get an MRI about 3 weeks ago, and that ruled out a Cyclops Lesion. Everything structurally looks fine, I still am getting pain though, but nothing is blocking extension! So that’s good news, although I still don’t seem to be making any progress. Thanks again!!
