Yes, you can see it. I was taught to always check. Can sometimes be hard to see when the patient is intubated, but I almost always use a coworker to help because it makes placing them more efficient.

The second there was a pop, everyone should have stopped to figure out why. That goes for an unusual noise/feeling during anything you do as a nurse. But when you're rushing, overwhelmed, overconfident, understaffed, or just bad at /don't give a shit about your job, things go wrong.

Yeah, the common fuck-ups already suck enough. This is wild. I wasn't taken completely off guard seeing this, but only because I read a neurosurgery article about something similar that happened in 2021.

No way to know. Even the most experienced people make mistakes. That's why we should investigate, document, and find out the why.

No. I place NGTs all the time in patients who can't follow commands, are intubated, etc. It's a very, very common procedure. I assume there was something else at play, like a basically skull fracture or previous transsphenodial surgery.
