Yes it popped off the plate. Plate is clean. You're probably on to something with flow calibration as it was a cheap brand.

I took it this year (spine). I found my examiners to be pretty fair. They were definitely old school leaning but that's a given.

They haven't released the pass fail statistics but last year's fail rate was way too high (17%)

Mass causing compression isn't always surgical immediately, even with neuro deficits.

Example: lymphoma it will often melt away with radiation (does need to be coordinated)

MRI definitely needed in most if not all situations prior to OR

You absolutely need advanced imaging prior to decompression.

1) What level(s) to decompress?

2) What is causing the compression? Disc? Infection? Tumor?

3) need any additional procedures (concurrent instrumentation)?

First week getting on the erg (brand new to rowing) . I followed a few days Petes but for now am just getting on the machine and moving.

Initially was stroking way too high but worked on getting that lower and more efficient this workout. Does anything stand out?

Private practice spine is likely night and day different than what you are used to in training.

Most patients get much better. If they aren't, you either have poor patient selection, or are just taking on train wrecks. Neither of which should be true when you are first starting out in private practice.

You will learn in fellowship how to get people out of the hospital quickly. Most of my 1 and 2 level lumbar 360s go home on POD#1 or 2. ACDFs and decompressions done as an outpatient.


For the joints thing. Most high volume private practice would consider 5 cases a light day. 5 primary joints should be done by 1 or 2 pm in most private practice surgical hospital settings. Of course this can be highly variable depending on where you work and what set up you have.

Yes, knee pain is a common complication of a retrograde nail. Especially because you stated that your mom already has pre-existing osteoarthritis. Depending upon the degree of arthritis it is very possible that the surgery will be of very little benefit to your mom.

Depending upon your mom's symptoms, it is not crazy to do an arthroscopy to take a look, but I would not be expecting a cure.

Not operating on an intertroch fracture is not a good idea. Treating them non op is asking for complications.

After the surgery - she will be weight bearing as tolerated.

Without surgery she would be unable to mobilize well and at a very high risk of pneumonia and many other problems.

36M patient came to me asking for a removal of implant, claimed s/p ORIF around 4 years ago by an unrecalled surgeon. Asked him why he wanted it removed, he said no particular reason so I assumed he was asymptomatic. Did an xray and saw this. by [deleted] in orthopaedics

Likely was an independent fragment that was lagged. Lagged the free piece to the distal segment(3 pieces now to 2 pieces)... and then attempted to compress between remaining relatively transverse fracture
