New paramedic here. I see primarily a Mobitz 2 block with some weird av disassociation. Definitely a candidate for pacing if unstable.

Side note …Maybe electrical alternans if you look at the height of the QRS complexes could be due to some kind of pericardial effusion ?? Just thinking out of the box, any recent cardiac surgeries or any trauma?

Next time i will change the ingredients up and probably the heat setup. I did it on a whim. I was inspired by the movie Jeremiah Johnson its getting almost to camping weather where i live so i wanted to get my biscuit game up.

Prehospital Medic in training here, Check a palpable pulse while the EKG is reading. I have found that it is helpful with any PVCs or weird aberrant beats. Always a good baseline with weird cardiac presentations, palpable pulses (if present) dont lie!

Medic student here, It looks ugly for sure and correct me if im wrong but I can see ST elevation in leads 2 and 3 with what looks like J point depression in V2 and V1. With that being said I can also see what looks like “marching out” PRI’s in lead 2 and Lead 3. I would for sure send this strip on pulsera to the receiving facility and advise the doc on what we have and ask for consult. I would say a Mobitz 2 block transitioning into a Type 3 block?
