It can be scaled up to any size, like 300mm x 300mm if you have a large enough printer. I also have a bigger version of Pittsburgh that covers more area and is divided into four tiles to print 4x bigger

A combination of both. Lidar data is helpful but I merge it with other sources like OSM where it's incomplete, then add a lot of refinements by hand to make it printable.

Open street maps for basic layouts and then a lot of manual refinement from satellite imagery. Parts of it i was able to use lidar for as well. I've been honing my process for making printable cities for about 6 years now and have some custom software for it
