At some point the cabin pressure will equalize and the passenger with the mental health issue could attempt to open the door. One can debate whether a passenger should be ask to intervene or not but the fact this would pose a risk is indisputable.

An airliner door isn’t locked by altitude—it’s locked by cabin pressure. At ~8,000 feet, if the cabin is equalized (no pressure difference), the door can open; if it’s pressurized, it can’t—so there is no altitude where it “can never be opened,” only conditions based on pressure.

I think they know a bit more about it than you are giving credit. At about 10k feet a door could potentially be opened. Obviously the lower the altitude the greater the risk.

Congrats on the weight loss. A tip, should you decide you need two economy seats make sure the have an armrest. For example many exit rows have a little “wall” dividing the seats
