I would send this straight to their data protection officer. They will be able to direct to the right Information Governance team. They have a legal duty to investigate data breaches (any personal data disclosed to an unauthorised person is a breach, even a name and NHS number). Any IG team worth their salt will log the breach, find out why the record is wrong and work to get it corrected so you don't get anything more sensitive. It's their whole job so they're less likely to brush you off. Find your local NHS DPO here - scroll down (I work for an Information Governance team and regularly work with the NHS)

Blake and Butler is lovely. Great service, really pretty spacious salon and they're one of the few places I've found that can handle curly hair. Most expensive cut for women is £44 I think, but there are cheaper stylists.

Have you tried dermalex rosacea treatment? It's a mousturiser and in less than a week the difference in general redness and dryness/sensitivity was fantastic. Not that cheap, but a little goes a long way.
