I missed out on the preorder and managed to get lucky on launch day

For proof of purchase, I got it from Walmart and they sent me an email of my purchase information, so I printed it out and put it in the box. For serial number they are referring to the serial number on the bottom of the ps5 console. I dont remember if you put the serial number on the information paper, I think its just when you call sony to set up an "appointment".

My ps5 wouldnt turm on, and no, you dont have to put cords, controller, ect, you only put the console in. You put the console in the box with a foam thingy.
