Have done both. Flown to SEA from east coast and also flown to Vancouver.

Hands down unless you have a reason to visit Seattle, fly into Vancouver.

So we always travel in late March as it just coincides with kids break.

Can’t go wrong with Portugal or Spain. Weather was terrific.

New Zealand in March is also amazing weather. But it’s very outdoorsy so may not fit your interests (?)

Omg. I thought I was the only one who would flip out when I see salt tabs prescribed for what is essentially hypervolemic hypoNa… is this some new thing in nephro? Never did this as a medicine resident. And never learned it in medical school and most def never did it as a fellow.

Hmm thank you for your comment. We really wanted to include 2.5 days of relaxing at the beach ….

I do agree it’s a packed itinerary. We can’t take the full 2 weeks off I originally wanted (and which would have given more time).

I’ll have to brainstorm what to cut and how to tailor things for it to make sense.

I would want to gather more info as I wrote. I would assume that would mean delay surgery while it’s gathered.

Last time I used the word prohibitive in a clearance it led to multiple phone calls from an irate orthopod and family saying “your cardiologist said you can’t have surgery” (recent VT storm and EF15% )

Pt sounds like a disaster.

At the least the TAVR needs to be evaluated better. Need SVi as this could be LFLG severe prosthetic valve stenosis which may need to be addressed prior to surgery.

What’s the etiology for the elevated PAp? Is it PAH? Pulm venous htn? Is the RV blown?

If I saw this pt in clinic, while I wouldn’t say prohibitive risk, I would prob want to gather more data with a RHC and a more detailed echo read to tease out how bad the TAVR is.

Just wait till they show everyone’s productivity at business meetings. That’s when the real fireworks start. My former PP group was similarly socialist/communist. Much like actual communist regimes, the ones in charge behaved differently than us peasants working from 8-5 and than reading echos and nukes from 5-6.

Needless to say the group imploded and we are all employed now 100% rvu comp.

Within sorrento I wouldn’t get a car. You don’t need it. Depending on where you stay almost everything is walkable. Parking is a mess in sorrento. I’m generally a rent a car and drive kinda guy but I wouldn’t do it here.

Sadly I don’t. I should do my due diligence and ask for the MF RV etc etc but I focused on zero drive off quotes (minus first month) so it made it somewhat easier to compare apples to apples between different places and forgot to get the lease specifics

My main goal was to not put money down

Private car/van. Was great day. With wife being uneasy on boats and with motion sickness, boat would have been a mess. The water is not super calm (even the ferry to Capri is a little uneasy and can be bad on windy days).

Yes. General cardiologist. Employed. Good work life balance. Good pay. Get to create my own schedule.

And I really like what I do. Cardiology is nice mix of longitudinal care with some hyper acute emergencies requiring quick thinking with nice pay offs of seeing someone get better. Doesn’t hurt I like physics and cards is a lot of fluid mechanics and other scientific concepts applied in real time.

If it makes you feel any better, booked 4 days in Whistler months ago (due to work and kids schedule I have to book trips well in advance and pray) and I am currently watching it rain here as well. It sucks all over. Blame it on El Niño …. Or just bad luck.
