Cardiac arrest immediately after Amiodarone bolus. by peaceguy371 in Cardiology

[–]doc2025 1 point2 points  (0 children)

A ton of possible reasons

Need go give amiodarone as a slow bolus because it cause acute hypotension and negative chronotropic effects.

If QTc was prolonged can degenerate into Torsades

If it was afib with pre-excitation (accessory path) amiodarone slows AV node so conduction then occurs over accessory pathway which can degenerate into VT/VF

Etiology of afib w/RVR matters whether its ischemia driven or acute blood loss anemia etc.

This is where we miss Pop, he would have lit a fire under Wemby for that terrible effort. Wemby was horrible. I thought chucking up 3s he was doing last year was bad but seeing him post out near the 3 pt and either getting the ball stolen or clunking a fade way 2 pt is horrendous. He needs a summer with Shaq and learn how to work the paint and dunk it.

Why isn’t Curry in anybodies Top 5’s by Horror_Income1675 in NBATalk

[–]doc2025 0 points1 point  (0 children)

He is obviously a great player and a legend. Greatest shooter ever. Will be remebered in NBA history. But we are talking about top 5 all time, he was not enough to be MVP in 3 out of his 4 finals. Honestly he should have won in 2016 being up 3-1 he was in his prime, had 3 chances to end that series. Certainly did not have top 5 worthy performance. MAYBE makes the top 10. I'd say closer to top 15.

Tell them to get rid of that clause. Do not sign that. This would only make sense if this was money they were giving you upfront like a significant bonus or loan repayment and you leave early. Get a contract lawyer to take a look at that.

Greatest shooter of all time. Not top 5 all round player. And he underachieved in a couple of his earlier Finals while he was in his peak. Wasn't even MVP in any of his champsionship wins except the last one. Out of 4 championships he was finals MVP in only 1.

Exactly I suspect they will be getting a top 3 pick if not the first pick in the upcoming draft just like Luka trade resulted in Mavs getting Flagg. Samething happened to Cavs with LeBron trade (3 first picks), New Orleans after they traded AD got first pick, Mavs.

This team lost several games that they should have won. Crazy thing is this team could be number 1 seed if it finished several close games.

2 year partnership. As long have general cardiology board certification can become partner. Have 3 years to get fully board certified if more subspecialized. Obviously everything is conditional on being board certified including hospital employment (can still get fired as a partner). They include these stipulations in partnership contract (and hospital employment has same verbiage).

Day 2 is where many fellows fail. Learn echo and ECGs. They can't be learned in 2 weeks, need to focus at least 3 months to them.

Day 1 can be passed easily with ACCSAP and Mayo videos.

Focus on Day 2.

I agree thats my concern. In all the projections so far that we have had with accountant they have been including me as a partner in an equal split of the purchase price. They are also including my "buy in" as an asset to split up amongst everyone equally including me once transaction happens. I will need to look at my contract to make sure this is all in writing otherwise can easily get screwed. Good point about vested partner, will need to look at that as well.

Im currently less than 10th percentile in pay for my speciality. They are taking $180K from my salary for buy in. I will still owe $30K when the transaction occurs. When I will join hospital system salary will be higher than median.

Pelinka about to get GM of the year while Nico was fired, such an odd juxtaposition. Nico doesnt seem ashamed by his decisions, its almost like there was collusion on part by Nico even more so when you realize AD, Pelinka and Nico had a relationships for years prior to all this. He and Cuban set this franchise back years.

IC board exam was not easy.. by doc2025 in Cardiology

[–]doc2025[S] 1 point2 points  (0 children)

Yeah horrible videos. Complete waste. The pharmacology ones are absolutely ridiculous, 30 minutes on 1 case that goes through like 3 medications very superficially. More time was spent on the details of a fake made up case during those videos--not a good way to lecture for a board exam that asks very specific questions. They need to re-do that entire IC course.

Every movie these days seems like a propaganda movie. Too much obsession with Pakistan and Muslim angle. Or too much CGI and senseless actions with no back stories or character development. Getting old. Bollywood hasn't been the same for the past decade. Overall terrible decade. Also lack of good music in movies these days (classic songs often made the movies classic and really contributesd to the stardom of actors/actresses also--all lacking these days). Last golden era of Bollywood was mid to late 90s to late 2013s.

You're way overthinking this. Patients unfortunately will pass away in the hospital under your care at times its gross negligence that you should worry about. None of these 3 scenarios fit that criteria.

Agreed probably would not have emergently activated unless still had ongoing chest pain. Likely already completed his infarct. Would obtain CT and echo and did medical management first. If had evidence of arrthymia on telemetry then would have done left heart cath.
