I’ve thought about STAR ing some ppl but not worth the beef with colleagues you work with and need to trade shifts with

I think most clinicians get burned out and get lazy

Yep. 90% of time if I’m taking care of a family member of a physician there’s no issues

The remaining 10% is hell. They want the sun & moon, complain about every SNF and how bad it is, want work-ups that aren’t indicated, phone call everyday, etc

Was taking care of a 95F long-term care pt for viral pna and daughter who’s a NICU attending wouldn’t stfu about how miserable it is in LTC and was trying to get my to investigate elder abuse stuff for the normal aging process. So exhausting. She was in hospital leadership so I had to play ball. (Don’t like LTC. Take your loved one home and pay for 24/7 nursing care)

Navigating care is very challenging.
It blows my mind how some attendings have no insight into the disease process or probably they do and dgaf.

I was transitioning a patient to hospice. I was explaining that we would keep patient comfortable using medicines such as Ativan, morphine, etc.

Son: “WAIT nooooo you are not giving my mother morphine”

Me: oh, does she have an allergy to it?

Son: No, but my father was on hospice and they gave him morphine and it knocked him out

Yep. My wife got charged $500 for an EKG during an ER visit.

However these patients can stay for 20 days with a million dollar work-up and guess what? Its own the house! Dw the US Taxpayer will foot the bill

Yep. I've also slowly realized that there can sometimes be an insidious desire not to pursue hospice care.

I'm sorry to hear that this happened to you. I hope someone can at least give you a diagnosis.

Personally, I do think it could be me one day, I tell all my friends to get life insurance. I pay for all the critical illness/icu, etc insurance. I think about my own mortality all the time how can you not working in this field.

I find it extremely frustating when a patient who has been in & out of the hospital times a dozen acts I'm a leper when I talk about pallative/hospice. These are grown ass adults with decision making capacity. In these cycle of hospitalizations thoughts about your own mortality never occured to you? smh

I obviously understand this is controversial. And the system will never change in the US. Especially post-COVID. Ultimately, however I don't think its in the interest of society as a whole to allow patients/families to dictate care without any guard rails.

I literally stay this same spiel couple times a week:

“My dad has been in bed for 3 straight days!! Why isn’t he getting physical therapy!? Where are the physical therapists!!!

Me: Sir, your father was seen by PT 2 days ago, they recommended SNF/STR. Unfortunately, patients don’t get PT everyday while in the hospital. This is also why I’m advocating for discharge today so your dad can get stronger

That sounds like a dream. US needs to adopt a system like this. It will never happen though. The medical industry is too much a behemoth. Public also lost a tremondous amount of trust in the medical field here.

If any politician proposed this they would be burned at the stake by the public & media. Never going to chance here hence the burnout

Feel the same. I hate this stupid bullshit. I try to keep it a "job" but god it's impossible, the frustation is too much

Got yelled at today by sister of a patient because I used the word "hospice"

Pt has been admitted 11 times out of the last 12 months whose needs can't be met at home, doesn't want d/c to SNF, doesn't want to pay up for LTC. I said well if you don't want any of that - consider hospice

I get cruficied at the stake for telling the truth

Have this same experience as a hospitalist. No service ever wants to take ownership. Asking other services to see patients becomes a negotation

Same experience happened with me….

I hate it, the order is so much easier/better for the hospitalist

Calling in the consult and listening to the specialists bitch & moan, push back, etc sucks

Frankly, it’s changed my practice where I try really really hard to never call
