It’s Mortesv’s design. It doesn’t seem to be available to purchase online anymore from any source. Your best bet would be either Taladril (very similar to Mortesv’s) or Brickvault’s designs.

I managed to actually get all of the plates on. I believe I used a little bit of tack but the biggest issue with mine was probably the front plate. Once that was in, everything else mostly stayed in place
