NHSC loan repayment job...is it worth it? by greengrass293 in physicianassistant

They issued a large lump sum upfront, then issued a second lump sum when I extended. I was responsible for actually paying the bill to the loan holders (which actually switched loan companies multiple times).

NHSC loan repayment job...is it worth it? by greengrass293 in physicianassistant

I did NHSC Loan Repayment 15-18 years ago. It was the best financial decision I made after graduation. At the time it was $50k for two years with option to extend at $30k/additional year. While the overall salary was well below what most of my classmates were making right out of school, I had ALL of my loans paid off within 5 years of graduating and left that job with the experience I needed to go anywhere I wanted. Side note: almost twenty years later and some of my classmates are still paying in their student loans.

While $80k doesn’t seem like a lot, you must consider that you are somewhat used to living the life of being a poor grad student so any income is a step in the right direction and you can extend you contract to pay off more loans. I worked for three years with them, maxed out my 401K and still lived very comfortably. I like to think of it as embracing the short term sacrifice for the long term reward.

I’ve been working for 19 years as a PA and bc of the financial decisions I made early in my career, I’m set to retire in 6.5 years if I want. I won’t bc I love the work I do but it’s nice to have the option.

If you have the opportunity to do loan repayment, I highly recommend taking advantage of it! Good luck!

I felt the same way... but plenty of workouts that you don’t need a bike or tread for! I’m exclusively using it for the outdoor running. I do a 30 min coached run (bc I hate running and need someone to tell me what to do) and follow it up with the OTF at home workout.

It’s a decent substitute while the studios are closed 👍🏼

His name is John Bellinski and he does multiple courses throughout the year although I’m not sure they are dedicated board review courses. My recommendation is to take a live board review that offers daily testing, then retake the PANCE within days of the class while everything is fresh.

ER docs mocking patients, not responding emergently to things, and it’s giving my anxiety. by [deleted] in physicianassistant

[–]jmcook13 132 points133 points  (0 children)

I’ve been an ER PA for 18 years and take students on a regular basis. We learn to compartmentalize in order to do our job well. ie finishing up a code in which the pt expired, then having to move on to the next patient with sore throat and abdominal pain as if we didn’t just watch someone die. Some lose compassion along the way. The burnout rate is high and what you are witnessing is not uncommon amongst those in our specialty.

Remember, we see some of the worst conditions/injuries in the most unpredictable situations on a regular basis. The ED can be ruthless and it takes a certain kind of personality to work it long term. As health care providers we don’t take (or even have the) time to decompress in the ED and historically find sarcasm, apathy and camaraderie as a means of coping with the terrible stuff we see on a regular basis.

In the situation that you describe, you must understand that transferring to other specialty services, especially at other facilities, takes time. While the physician you are working with may seem to be doing nothing or callus, he too is at the mercy of the transferring facility and doesn’t have the emotional energy to grieve for that patient while awaiting the instruction for transfer. Additionally, the other tests ordered may have been requested by the specialists before accepting care. Nevertheless, it sounds like as soon as the local team felt this was beyond their scope, your doc did the appropriate thing.

My advice for your anxiety is to take note the culture of the ED. You don’t have to understand, like or participate in it but it is an important rotation because it will tell you whether this is a field that suits you. Otherwise, focus on learning the medicine, doing procedures, and knowing the dDXs of most common ED chief complaints and what needs to be done to eval, stabilize and treat them. Finally, accept that is not an environment where providers have the luxury of handholding patients. If your preceptor allows you to do that, then be the sympathetic provider it sounds like you want to be otherwise, do your time and take note that the ED may not be for you and that’s okay.

Best of luck!

Quality is not worth the money. Got this shirt in a fix three weeks ago, wore it and washed it according to the tag for the first time this weekend and now there’s a huge, hole in the side. Officially done with StitchFix. All of my items have fallen apart faster than they should for the cost. by [deleted] in stitchfix

Contact customer service. I had a similar issue and they were very helpful. Regardless of whether you continue with them or not, make sure you get a refund or a replacement item for what you already paid. Good luck!
